A plan reassessment can be actioned by you as the participant, or nominee of a participant. Since July 1, 2022, the NDIS has been offering plan continuations for many participants, if their plan is working well for them.
If you are (or your child is) expecting a life changing event, for example moving from primary school to high school, leaving school, leaving home etc., you can request a reassessment.
To prepare for a reassessment, Kelly from Grounded Earth Mumma suggests collecting the following information, with the help of your Support Coordinator if you have one:
- Occupational Therapy report (and any other therapists you use)
- Any quotes for high-cost items you expect you will need to support your goals – the quote needs to reflect something that is mentioned in the therapist report and needs to be supported by this kind of evidence
- Write and submit a carer’s impact statement – here’s an example Writing a Carer Statement – Association for Children with Disability (acd.org.au)
- Ask your support worker/s to supply a statement (need link/example)

Important: After the reassessment meeting, ask for a draft of the new plan before it is submitted.
Check the information, including goals, is correct – and make sure it is consistent. You can ask your Support Coordinator for help with this, or an advocacy service.
If you receive a new plan, please let your Plan Manager know so they can update their system.
