The SC Portal is changing, so what do I need to know? 

  1. Login – We have a new login page, that looks like this 
    and can be found here https://portal.plantracker.com.au, 
  2. Password – You will need to create a new password to log into the portal. The password must be 8 characters and include a number and a letter. You must use the login link sent to you via your email to set up the password. Do not use the forget my password option on the login page 
  3. SC Portal App – there is now an app so you can access your information from your phone or tablet on the go. 
  4. Participant View – You can now view the same information as your participants, and toggle through the SC Portal to the participant portal- great to help your participants with budgeting and keeping up to date with invoices 
  5. Through the participant view – You can see invoices and their status, to track that an invoice has been paid or if there is a potential issue. 
  6. Through the participant view – You can download a copy of the original Invoice or remittance advice for any or your participant’s providers. 
  7. Through the participant view – You can view Service Agreements and see what funds are allocated, what’s been spent, or support your participant to release funds.  
  8. Paper NDIS plans – paper plans will not be stored on the SC portal, however a copy can be obtained through the ‘Ask a question’ 
  9. This is Phase 1 of our upgrade of the SC Portal so some functions have been removed temporarily such as the “My Team” screen and the toggle between tiles and list view. These functions will return – new and improved – in 2023. 
  10. Provider Portal – We also have a new Provider portal, so your participant’s providers can upload and track invoices. Or if you are the business owner, keep track of all your participant’s support coordinator invoices.
